Diabetes Prevention Program: 

Congratulations to the VCHD Health Educators for receiving Full Plus Recognition from the CDC for our Diabetes Prevention Program! This means the program has effectively delivered a quality, evidence-based program that meets all standards for CDC recognition.

86 million American adults- more than 1 out of 3- have pre-diabetes. 90% do not know they have it.

This virtual programs offers:

  • Easy tracking of weight, activity, food, sleep, and stress
  • Program material via web or app
  • Access to community support and a lifestyle coach 24/7
  • Free, fresh produce to those who qualify
  • Free YMCA membership for those who qualify

In order to enroll in the program all participants must meet the following criteria:

  1. All participants must be 18 years of age or older and not pregnant at the time of enrollment. These programs are intended for adults at high risk for developing type 2 diabetes.
  2. All participants must have a BMI (body mass index) of ≥25 kg/m² (≥23 kg/m², if Asian American).
  3. All participants will be considered eligible based on either:
    • a recent (within the past year) blood test meeting one of the following specifications:
      • Fasting glucose of 100 to 125 mg/dl
      • A1c of 5.7 to 6.4
      • Clinically diagnosed gestational diabetes mellitus (GDM) during a previous pregnancy; or
    • a positive screening for prediabetes based on the CDC Prediabetes Screening Test.
  4. Participants cannot have a previous diagnosis of type 1 or type 2 diabetes prior to enrollment.
  5. Participants must be an Indiana resident
